Change of name and address details
Registered nurses have a legal responsibility to notify the Board of a change of address within 14 days of that change having been made. Failure to do so is an offence under the Health Professions Registration Act 2005 and may be subject to a substantial penalty. The Change of Address form should be used by registered nurses to notify the Board of a change of address, whilst the Change of Name form should be used to notify the Board of a change of name.
Authority to Act
This form is for applicants that wish to nominate an Agent or other person to 'Act on your behalf' to transact business in all matters pertaining to your application for registration as a nurse in Victoria with the Nurses Board of Victoria.
Certified Copies of Documents
The Board accepts copies of documents in lieu of originals when the copy has been correctly certified by an authorised person approved by the Board.
