Change of name and address details

Registered nurses have a legal responsibility to notify the Board of a change of address within 14 days of that change having been made. Failure to do so is an offence under the Health Professions Registration Act 2005 and may be subject to a substantial penalty. The Change of Address form should be used by registered nurses to notify the Board of a change of address, whilst the Change of Name form should be used to notify the Board of a change of name.


Authority to Act

This form is for applicants that wish to nominate an Agent or other person to 'Act on your behalf' to transact business in all matters pertaining to your application for registration as a nurse in Victoria with the Nurses Board of Victoria.


Certified Copies of Documents

The Board accepts copies of documents in lieu of originals when the copy has been correctly certified by an authorised person approved by the Board.

Overseas qualified nurses

This form is for applicants who have gained their nursing qualification overseas (excluding applicants holding a current practising certificate in New Zealand). Download Form (130) kb

Mutual Recognition

Victorian Graduates

This form is for Victorian students of nursing who have completed courses accredited by the Board leading to registration in division 1 or 2. Download Form (112) kb

Nurses without current registration in an Australian State/Territory

This form is for applicants who have gained their nursing qualification in another Australian State/Territory, however do not hold current registration in their State/Territory. Download Form (130) kb

Australian Defence Force personnel

This form is for applicants who have gained their medical/nursing qualification in the Australian Defence force and may be eligible for registration in division 2. Download Form (130) kb

Recognition of Additional Qualifications - Division 1

This form is for applicants who are currently registered or applying for registration in division 1 and have gained a post graduate qualification either accredited by the Board or a qualification equivalent to a Board accredited course in psychiatric nursing, maternal and child health nursing or nurse immunisation. Division 2 medication administration endorsement

This form is for applicants who are applying for registration or are currently registered in division 2 who may be eligible for endorsement in medication administration. Prior to submitting the form please read carefully the Checklist of Requirements and Information Sheet included in the application form to ensure that you have the qualifications required. Temporary Registration (restricted registration)

This form is for applicants who wish to undertake post graduate nursing studies in Victoria and require registration to undertake the clinical component of their course.Download Form (115) kb

Re-Registration of Temporary Registration (restricted registration)

This form is for applicants who require an extension of their temporary registration in order to complete their post graduate nursing studies in Victoria.Download Form (92) kb

Applicants currently or previously registered in division 3 or 4 only

This form is for applicants wishing to apply for registration in division 1 on the basis of their current or previous registration in division 3 or 4 only. Download Form (130) kb

Midwifery Endorsement

This form is for applicants who are currently registered or applying for registration in division 1 and have gained a post graduate qualification either accredited by the Board or a qualification equivalent to a Board accredited course in midwifery. Download Form (94) kb

Application for Statement of Registration to Replace 2008 Renewal Card

From 15 April 2008, the Board will no longer be able to issue replacement 2008 renewal cards. If you have not received your 2008 renewal card or have misplaced it, the Board will issue a Statement of Registration free of charge which is official proof of your registration until 31 December 2008.Download Form (51) kb

Application for a replacement certificate or statement of registration

Do not use this application to apply for a replacement renewal card. Please click here for the correct application form.

In the event of loss, destruction or non receipt the Board may only issue a replacement certificate within three (3) months from the original date of issue. Thereafter a statement of registration may be issued confirming the registration status.Download Form (61) kb

Application for verification of nursing course and registration
